相關時程如下：
一、預約登錄截止期間：115年4月27日。
二、健康檢查截止日期：115年4月30日。
三、為利作業安排，請於檢查日期前4天完成預約登錄。

一、請假類別請點選「公假」，並選擇「健康檢查公假」。
二、並請附上相關佐證文件（如：本校健檢預約登錄畫面截圖、健檢預約掛號證明等），以利後續作業。

Important dates are as follows:
- Health check deadline: April 30, 2026
- Appointment registration period: October 28, 2025 to April 27, 2026
- To allow for administrative arrangements, please complete your appointment at least 4 days prior to your selected check-up date.
