為維護本校教職員工健康，並配合辦理 114／115 年度教職員工健康檢查作業，敬請尚未完成健康檢查之教職員工，利用寒假期間前往高雄醫學大學附設醫院 健康檢查中心完成健康檢查。
相關說明如下：
一、健檢期間：114年11月1日~115年4月30日
二、預約時間: 預約登錄時間:114年10月28日起至115年4月27日止，並於檢查日期前 4 天截止登錄，以利醫院前置作業。
三、預約登錄時間:114年10月28日起至115年4月27日止，並於檢查日期前 4 天截止登錄，以利醫院前置作業。
四、自費加選內視鏡檢查、電腦斷層檢查、正子造影檢查者，需於健檢前20天至本網頁預約登錄並自行打電話跟醫院預約確認。
五、健檢地點：高雄醫學大學附設中和紀念醫院健檢大樓(C棟)2樓健康管理中心4號櫃台)
六、醫院地址：高雄市三民區自由一路100號C棟2樓。
七、請自行上網預約登錄本校「學務處健檢預約登錄網」完成登錄，每日健檢預約上限為20名，如未事先預約登錄，自行至醫院檢查者 ，醫院當日無法受理健檢，詳請參閱健檢懶人包中英文版
八、本校健康檢查系統登錄網址及帳號、密碼：https://sis.nsysu.edu.tw/SMMS/staff/staff_index/index.php，登錄帳號和密碼:帳號為身份證字號且須為本校教職員工，密碼:身分證字號(員工)前2碼(含英文字)加

Dear Faculty and Staff,
To safeguard the health of all faculty, staf, and in accordance with the
implementation of the 2025–2026 Health Examination Program, those who
have not yet completed their health examination are kindly requested to make
use of the winter break to visit the
Kaohsiung Medical University Chung-Ho Memorial Hospital – Health Examination
Center to complete the required health examination.
The relevant information is provided below:
